How can I manage my Monument membership or make changes to my treatment plan?

To change your treatment plan or enroll in a personalized plan, log in to your Monument account. Select the settings button (your initials in a circle) in the top right corner, and then select 'My Care Plan' on the left-hand navigation. You can manage your membership at any time.
