To change your treatment plan or enroll in a personalized plan, log in to your Monument account. Select 'Account' in the top right corner, and then select 'Membership' in the left-hand navigation.
If you're currently enrolled in Community Membership (Free), you can select 'Subscribe To Premium' to take the pre-screener survey, see your options for personalized treatment, and enroll in the plan that's best for you.
If you're currently enrolled in a personalized treatment plan (Total Care Bi-weekly, Total Care Weekly, or Physician Care), you can change plans by selecting the 'Change Plan' button. You will then be able to select a new plan, including Community Membership (Free). You can cancel or change your treatment plan at any time. If you are changing to a lower cost plan, you will be refunded for the remainder of the month based on the number of clinician appointments you have attended. If you are changing to a higher cost plan, you will be charged for the cost of your new plan and refunded for the remainder of your previous plan month based on the number of clinician appointments you have attended.
